Enhanced Primary Care Plan / Chronic Disease Management

What is an Enhanced Primary Care Plan (EPC) / Chronic Disease Management Plan (CDM)?

An EPC is a scheme provided by Medicare that allows individuals who have a chronic condition (6 months or more) to access allied health services as part of the management of their chronic condition. Medicare benefits are available for up to five allied health services per patient per calendar year. 


How do I access the Medicare Rebate for Exercise Physiology?

To access the Medicare rebate, you must first attend an appointment with your GP discuss the EPC and acquire an appropriate referral for Exercise Physiology at a specific clinic.
