Utilising Home Care Package funding for Exercise Physiology assists seniors to live independently within their own homes.
Home Care Packages (HCPs) provide services for seniors that sit between the entry level Commonwealth Home Support Programme and residential aged care for seniors who are no longer able to live at home.
A HCP is a coordinated package of care and services to assist seniors in living independently at home for as long as possible. There are 4 levels of HCPs, ranging from basic to high level care needs, which is determined by an assessor.
These funds can be used for personal service, transport, assistive technology, meal preparation and allied health services such as Exercise Physiology.
Once a HCP is approved they are able to sign up with a provider that best meets their needs. The HCP provider will assess the clients’ needs and refer service providers such as Exercise Physiologists. The HCP managers or clients can schedule assessments and services with The Sydney EP.
